What is a facility fee on a medical bill?
Why a visit to a hospital-owned clinic can produce two charges, what a facility fee pays for, and what to ask before and after the visit.

A facility fee is a charge from the hospital or health system for the place where you were seen: the room, equipment, nursing staff and overhead. It is separate from the professional fee, which pays the doctor or other clinician.
Why you might see one
- Hospital-owned clinics. Many doctors' offices are owned by hospitals and bill as a hospital outpatient department. A routine visit there can produce a facility fee plus a professional fee, where an independent office would bill once.
- Emergency rooms and outpatient procedures. The hospital bills for the room and resources, and the clinician bills separately.
- Telehealth from a hospital system. Some systems charge a facility fee for virtual visits too.
How to recognise it
On an itemized bill it often appears as a clinic or outpatient charge with a revenue code, on the same date as a professional charge. On your insurer's explanation of benefits it can show as a separate claim from the hospital rather than the doctor.
What to ask
- Before a visit: "Is this location billed as a hospital outpatient department, and will there be a facility fee?"
- After a visit: "Which line is the facility fee, what does it cover, and how was the amount set?"
Some states limit facility fees for certain services. Your state's health or insurance department can tell you whether a rule applies to your visit.
